Overview

Create Allied Health provides expert housing navigation for complex cases where standard housing services have been unable to achieve outcomes. Our clinical social workers bring clinical assessment skills, crisis intervention expertise, NCAT application experience, guardianship knowledge, and professional advocacy to help clients secure and sustain safe, appropriate accommodation.

Crisis Housing Intervention

When a client faces immediate homelessness, eviction, or unsafe living conditions, we respond within 48 hours to provide:

  • Rapid clinical assessment of housing needs and risk factors
  • Clinical stabilisation and immediate safety planning
  • Emergency placement coordination with crisis accommodation providers
  • Advocacy with housing services and government agencies

NCAT Housing Applications

We have extensive experience with NSW Civil and Administrative Tribunal processes for housing and accommodation matters, including:

  • Guardianship applications for accommodation decisions
  • Capacity assessment to support tribunal applications
  • Tribunal preparation including evidence gathering and report writing
  • Coordination of clinical evidence from treating teams

Complex Housing Navigation

For clients who face multiple barriers to securing housing, we provide hands-on support through every stage of the process:

  • Attendance at property inspections with the client
  • Professional references and clinical support letters for applications
  • Tenancy education and skills building
  • Sustainment follow-up after move-in to prevent tenancy breakdown

Social Housing Applications

We support clients applying for priority social housing through:

  • Priority housing advocacy with clinical evidence
  • Professional support letters documenting housing need and clinical circumstances
  • Application preparation and submission
  • Follow-up and escalation with housing providers

SDA/SIL Provider Matching

For NDIS participants requiring Specialist Disability Accommodation (SDA) or Supported Independent Living (SIL), we provide:
